An online tool that assists Maine residents to assess their risk for chronic disease, receive feedback that promotes action to address identified health needs, and link them to local community resources. There are two main components: a confidential assessment that provides a personalized report and a searchable database of low cost/no cost health care services as well as information about how to obtain affordable health insurance. This tool is especially helpful for those who are uninsured, underinsured, or experiencing health disparities. Maine CDC partnered with 2-1-1 Maine, which maintains a database of low-cost community supports and programs.
